The CEO's office requests that the following CEO Checklist be used when making contact with the CEO's office with a view to requesting his signature on research-related documentation, including contracts.

The procedure is for the on-site contact person for the study (usually a research nurse or consultant) to make contact with the Hospital Financial Controller Once financial arrangements have been agreed, a letter from the Financial Controller will issue.

The Financial Controller's letter, the CEO Checklist along with enclosures and documents for signature should then be forwarded to the CEO's office.
